    COVID-19 Vaccination Reporting

    Individuals who receive the COVID-19 vaccine may experience side effects and the side effects could be consistent with COVID-19 symptoms. Please, stay home if you feel unwell after receiving your vaccinations and contact a healthcare provider for further direction.       

    The district would like to proactively monitor when staff members are vaccinated for COVID-19 in order to watch for any potential staffing issues should a large number of employees experience side effects at the same time.            

    Submission of your personal vaccination information is voluntary and will be kept confidentially in Human Resources. Total numbers of employees broken down by building/department will be the only information shared with principals/leadership team members.

     Employee Recognition

    The Schuylkill Valley School District values its employees and the contributions they make on a daily basis towards the mission, vision, and shared values of the district. Employees exhibiting exceptional individual or group contributions to the betterment of the district or our community deserve to be publicly recognized for their efforts. Please use this form to recommend a district employee (teacher, paraprofessional, substitute, coach, etc.) for formal recognition of excellent performance.

    Anyone is welcome and encouraged to recommend any district employee for formal recognition.

    Employees receiving a recognition recommendation will have their name entered into an annual drawing for an additional personal day. Employees will have their name entered one time for each recommendation so employees receiving multiple recommendations during the fiscal year will have their name entered into the drawing multiple times.

    The annual drawing will be conducted on July 1st of each year and will include all recommended employees from the previous fiscal year (July 1st - June 30th).

    Telework Request Form

    Please submit a request for a telework arrangement by using the following link.  Telework requests must be submitted fourteen days calendar days prior to the proposed start date of a telework arrangement or as soon as is possible if fourteen calendar days' notice cannot be provided.

    Approval or denial of a telework request will be emailed to you as soon as is possible after your request has been reviewed with your supervisor.  Approval of a telework arrangement will be accompanied by a telework agreement that will be signed by you and the district.
