  Individuals who receive the COVID-19 vaccine may experience side effects and the side effects could be consistent with COVID-19 symptoms. Please, stay home if you feel unwell after receiving your vaccinations and contact a healthcare provider for further direction.
   
  The district would like to proactively monitor when staff members are vaccinated for COVID-19 in order to watch for any potential staffing issues should a large number of employees experience side effects at the same time.
   
  Submission of your personal vaccination information is voluntary and will be kept confidentially in Human Resources. Total numbers of employees broken down by building/department will be the only information shared with principals/leadership team members.

  Click "Done" to submit your survey and then email Robin Brightbill to schedule a brief meeting to document that you have been vaccinated as well as to obtain the period of time that you will not need to quarantine if you are exposed to someone who has tested positive for COVID-19.
